Abstract: Ischemic stroke of the middle cerebral artery (MCA) is a major cause of motor impairment and frequently requires intensive rehabilitation. In this stratified randomized controlled trial, we investigated training-induced structural brain changes using magnetic resonance imaging (MRI) in stroke patients undergoing exoskeleton-assisted rehabilitation, while the control group received conventional rehabilitation therapy. To assess microstructural white matter alterations, we implemented a lesion-informed diffusion tensor imaging (DTI) processing pipeline. Changes in fractional anisotropy (FA) and mean diffusivity (MD) were analyzed using linear mixed-effects models to compare both groups over time. This study contributes to a better understanding of neuroplastic mechanisms underlying motor recovery and may inform the optimization of exoskeleton-assisted rehabilitation strategies after stroke.
